Overdose Data to Action (OD2A) is a 5-year cooperative agreement designed to provide local jurisdictions with resources to improve surveillance efforts to inform prevention activities and ultimately reduce the opioid burden.

Provides communicable disease screening and referral for follow-up medical care for refugees.

Provides services to persons suspected of or diagnosed with TB.

Provides case management of in-home services that assist eligible disabled adults to live safely in their homes with a support system and avoid placement in an institution.

Engaging and training members of faith-based organizations, small businesses, and community partners in strategies to prevent chronic disease through a faith-based community engagement initiative. 

Helping nurses and nutritionists provide preventive, consultative, and direct health services during the school day for students attending Charlotte- Mecklenburg Schools (CMS). 

Assures accuracy and integrity of patient record information and responds to request for copies of patient records. 

A community effort founded to slow the spread of HIV in Mecklenburg and the surrounding counties.
