Physical Exams Required For Pre-K and Kindergarten

Girl Reading bookPre-K students registering for school need a physical exam done within the 12 months prior to  entering Pre-K.

Kindergarten students need a physical exam dated within 12 months prior to  entering kindergarten.
 
A Kindergarten Health Assessment and other required forms are available.
 This completed form must be submitted to the school when registering for admission. 
 
For questions regarding this form, please contact your doctor or the School Nurse at your child's school.
