The Community Social Work (CSW) unit performs home visit and provides services, referrals, long term planning and case management to at risk families, senior citizens and adults with disabilities that will aid them in maintaining themselves safely in the community. Those services may include In Home Aide, Adult Day Care, Home Delivered Meals, and Representative Payee services. Community Social Work also provides Respite services to family caregivers to support their efforts to keep their loved ones at home. Most programs are not based on income, but priority is given to those with greater health and professional care needs.
