Who is eligible?
Children ages 0-5 who are experiencing one or more of the following:
- At risk for or diagnosed with a long-term physical, developmental, behavioral, or emotional condition
- In Neonatal Intensive Care Unit (NICU)
- In foster care
- Exposed to long-term stressful situations
