Case Management for High Risk Pregnancy (CMHRP) is a statewide program in North Carolina promoting healthy mothers and healthy babies. This program provides care management services for high risk women during pregnancy and for 2 months after delivery by a social worker or nurse. Referrals are made to our program to provide additional support during pregnancy.

Free Services to Support You and Your Baby:
Our nurses and Social Workers help you:
- Identify your needs and make a plan to address them
- Talk to your child's doctor or nurse
- Get transportation to medical appointments
- Get into services like WIC, Medicaid, and family planning
- Get education and support
You may be eligible if you
- Have Medicaid or are Medicaid-eligible
- Need help to guide you through the process
- Need help getting into services like WIC, Medicaid, and family planning
- Have needs identified by your doctor, such as
- Are pregnant with multiples (twins, triplets)
- Have a history of pre-term labor (less than 37 weeks)
- Have a history of low birth weight (less than 25 grams/5 lbs., 8 oz.)
- Have chronic medical conditions or behavior health issues that may complicate your pregnancy
- Have an unsafe living environment (homelessness, inadequate housing, violence/abuse)
- History of substance abuse or tobacco use
- Have entered into prenatal care late (after 14 weeks)
- Miss two or more prenatal appointments without rescheduling
- Have hospital visits (Emergency Rooms, labor & delivery triage)
- Have trouble with basic needs (food, utilities, transportation)
- Have fetal complication
