About CAP/C
The CAP/C Program provides services for medically fragile children under 21 who are at risk of institutional care. By providing in home nursing care, case management, and other supports, CAP/C can help these children stay at home with their families.

- Durable medical equipment
- Medical supplies
- Disposable incontinent products for children age 3 and older
- Home mobility aides (wheelchair, ramps, doorway widening, safety rails, non skid surfaces, hand held showers and grab bars)
- Pediatric nurse aide care
- Home modifications
- Vehicle modifications
Eligibility
Children must be under the age of 21 years. Clients must also qualify for CAP/C Medicaid at Department of Social Services.
