Non urgent reports can be made outside of normal business hours, on weekends or holidays by leaving a message on the Communicable Disease Line (980) 314-9201.  Please provide a contact name and number for the CD Nurse to call you back the following business day.

Urgent reports. Should you need to alert us of an IMMEDIATE PUBLIC HEALTH THREAT outside of normal operating hours specifically as it relates to suspicion or confirmation of the conditions or events noted below, the Emergency CD Line 704-432-0871 will be answered immediately or will return your call within 15 minutes.: 

·         Novel and/or highly infectious diseases (measles, mumps, Ebola, Middle Eastern Respiratory                  Syndrome)

·         Biologic toxins, hazardous chemicals or mass casualties

*Please do not call this line to ask general communicable disease questions and/or request appointments as our team needs to be able to respond to IMMEDIATE PUBLIC HEALTH THREATS in a timely manner.  

Callers who have questions regarding monkeypox and/or monkeypox vaccine should call 980-314-9400.  Calls will be answered and/or returned during normal operating hours.

What diseases are reportable?

Reportable Disease List for North Carolina

Suspected cases of diptheria, measles, polio, rubella, or congenital rubella syndrome (CRS) must be reported immediately.  Suspected cases of mumps or pertussis involving high-risk settings (such as healthcare settings and childcare settings providing care to infants) must be reported within 24 hours.


Who must report?

Physicians, school administrators and child care operators, medical facilities and operators of restaurants and other food or drink establishments (G.S. § 130A-135 through 130A-139) must report cases or suspected cases of reportable diseases to their local health department.


Diseases Reportable within 24 Hours ​​

Telephone reports are requested within 24 hours for diseases of unusual significance, incidence, or occurrence which may merit an epidemiological evaluation including foodborne and waterborne outbreaks where a common source is suspected.

For diseases and conditions required to be reported within 24 hours, the initial report shall be made by telephone to the appropriate person listed on theStaff Directory.

A NC Communicable Disease Report Form should also be completed and faxed.

 

Telephone reports should include:​

  • disease
  • date of onset
  • name of person making report
  • telephone number of person making the report
  • laboratory confirmation, if available​
  • patient name
  • patient address
  • ​patient age
  • patient sex
  • patient race

Diseases Reportable Within 7 Days

For diseases and conditions that must be reported within 7 days, a NC Communicable Disease Report Form ​​should be completed and mailed to the appropriate person and Health Department site.​​ ​​
